Smith is of African-American descent and was raised in the Hollis section of Queens. He is the fifth of six children and graduated from Thomas Edison High School in 1986. Smith received a basketball scholarship to attend Winston-Salem State University, an HBCU in North Carolina. He graduated with a degree in Mass Communication in 1991.

Smith was a writer for The Philadelphia Inquirer in 1994. He was an NBA columnist for the Philadelphia 76ers when he got the chance to become a general sports columnist. In April 2005, Smith became a radio host on WEPN. He began hosting Quite Frankly with Stephen A. Smith on ESPN. After its cancellation, Smith was an NBA analyst and left ESPN in 2009. He returned to ESPN in February 2011 as a columnist for their website and weekday radio show host. In 2012, he joined First Take as a commentator and, in late 2014, signed a multi-year deal worth $3 million yearly with ESPN. In 2019, Smith became a UFC commentator.
