Fillers have a branding problem, often associated with overfilled cheeks and swollen faces. But that reputation has more to do with outdated technique than modern practice, says Anthony E. Brissett, MD, a Houston-based facial plastic surgeon. Used correctly, fillers aren't about chasing a line or adding volume, but about restoring support in places most people don't immediately notice, reinforcing structure so the face ages better over time.
Why it matters
Understanding how fillers work can clear up the confusion many men have about their use. Fillers can replace lost structure, redistribute light to soften shadows, and reinforce shape and geometry without making the face look artificial or overly plumped.
The details
There are three main types of fillers: hyaluronic acid (HA) fillers, biostimulators like Radiesse and Sculptra, and fat grafting. HA fillers provide immediate structure and precise control, while biostimulators trigger the body's own collagen production over time. Fat grafting uses the patient's own fat to restore volume. The key is using these tools thoughtfully to restore balance and support across the face, rather than just filling individual features.
- Fillers can start being used in the 30s as the face begins to lose bone, fat, and connective tissue.
- Rapid weight loss, particularly from GLP-1 medications, can also create a need for more thoughtful filler use to address sudden facial fat loss.
