Medicare Introduces APCM in 2025, Creating New Monthly Revenue Stream for Primary Care Physicians

Advanced Primary Care Management (APCM) is Medicare's most significant primary care billing update since the introduction of Chronic Care Management in 2015.

Published on Mar. 10, 2026

Medicare's 2025 APCM billing update pays primary care physicians a monthly per-patient fee for care management, with no time-tracking required. APCM is the biggest primary care billing opportunity since 2015, and Tile Health automates the entire workflow so physicians can capture it without the paperwork.

Why it matters

APCM represents a significant new revenue stream for primary care providers, as it bundles elements of three existing programs into one simplified payment model. Unlike its predecessors, APCM is status-based, not time-based, eliminating the administrative burden of documenting every minute of care time.

The details

APCM covers patients with zero or one chronic condition (reimbursing $16.70 per month), patients with two or more chronic conditions ($53.91 per month), and patients who qualify for Level 2 and are also Qualified Medicare Beneficiaries ($117.53 per month). The billing provider must be the continuing focal point for all of the patient's primary care services, and only one practice can bill APCM for a given patient in any calendar month.

  • APCM was introduced by CMS effective January 1, 2025.
  • The 2026 national average reimbursement rates are: G0556 ($16.70 per month), G0557 ($53.91 per month), and G0558 ($117.53 per month).
