LCDHD Seeking Nominations for Public Health Hero Award

Annual award recognizes outstanding contributions to community health

The Lake Cumberland District Health Department (LCDHD) is now accepting nominations for the Dr. Hossein Fallahzadeh Public Health Hero Award, an annual honor recognizing individuals who have made exceptional contributions to improving and protecting the health of communities across the Lake Cumberland District.

Why it matters

This award celebrates public health champions who demonstrate exemplary leadership and dedication in promoting community wellness, supporting public health programs, and positively impacting overall health outcomes in the region.

The details

Nominees must live and/or work within the Lake Cumberland District, and anyone can submit a nomination to recognize a deserving public health advocate. Past recipients include long-time local physicians, a Family and Consumer Sciences Extension Agent, and other professionals who have made remarkable contributions to community health.

  • Nominations are being accepted now through February 28, 2026.
  • The award recipient will be announced during Public Health Week in April.
