The Grayson County Healthcare Foundation has awarded a nearly $95,000 grant to the Grayson County Health Department to establish a Community Health Worker (CHW) program. The program aims to strengthen the community's health and expand essential services to local residents by hiring and training CHWs to serve as trusted connectors between residents and the resources they need.

The details
The $94,210 grant will allow the health department to hire and train CHWs who will work directly with individuals and families to identify barriers, navigate available services, and link them to programs that support physical, mental and social wellbeing. As part of the initiative, the health department will also offer the Kentucky Prescription Assistance Program (KPAP) to help residents access necessary medications.
